If you feel that any member of our personnel has failed in respect to the policies of the agency, has acted out in an act of Abuse, Neglect, Exploitation, or has violated your rights in any way, please DO report it immediately to:

Most of the problems can be solved at this level.

Administration will investigate the problems within 5 days of receipt and will resolve it as soon as possible, but no longer than 30 days. On reports of Abuse, Neglect or Exploitation, the agency will investigate within 24 hours, report the problems to DADS immediately (within 24 hours), and submit written resolution to DADS within 10 days — according to regulation. If you do not feel satisfied with the action that took place, you can report it further by calling the Home and Community Support Services Licensing Division:
